You should use Trac links when referring to pages in Trac. So change:

[https://trac.macports.org/wiki/SummerOfCode2015_portfile]

to:

wiki:SummerOfCode2015_portfile


On the SummerOfCode2015_portfile page, you also have link to a Trac page, which should be changed from:

https://trac.macports.org/browser/branches/gsoc15-portfile

to:

browser:branches/gsoc15-portfile
