#21217: Move conf/extras-conf/mod_php.conf to conf/extra/httpd-php.conf

 When installing the php5 and apache2 ports (a pretty common
 configuration), you end up with the PHP configuration in
 /opt/local/apache2/conf/extras-conf/mod_php.conf. This is inconsistent
 with (most of) the other ports with offer apache2 integration which put a
 file named httpd-foo.conf in /opt/local/apache2/conf/extra/. This
 inconsistency is confusing for users.
