 The CoreAudio syntax changed.  I'm hoping that rather than using the old
 SDK we can just fix the glitch by updating the syntax in the OSX output
 module with a patch.  I committed such a patch in r57514 and would like to
 know if it works for you; if so, we probably want upstream to remove the
 hack.
