 Please remember to Cc the port maintainer.

 To build a port universal, you must build its dependencies universal -- in
 this case, cairo and libpixman.

 The correct command to rebuild libgdiplus and all its dependencies
 universal is:

 {{{
 sudo port upgrade --enforce-variants libgdiplus +universal
 }}}

 Please try that.
