 I have used for years this configuration in macports.conf :
 proxy_override_env    yes
 proxy_rsync       localhost:3128

 and an ssh tunnel on localhost 3128 to an external server running squid5.

 Since a few weeks, macports tries to connect directly when doing a
 selfupdate and ignore the proxy settings.
