I don't see where you mention which perl you have.

bash-3.2# port installed perl*
The following ports are currently installed:
   perl5 @5.8.9_0 (active)
   perl5.8 @5.8.9_3 (active)

bash-3.2# port installed mysql5*
The following ports are currently installed:
   mysql5 @5.0.81_0
   mysql5 @5.1.41_0 (active)
   mysql5-server @5.1.41_0 (active)

bash-3.2# port installed p5-dbd-mysql
The following ports are currently installed:
   p5-dbd-mysql @4.013_0+mysql5 (active)

Mac OS X 10.5.8 Intel.
