openssh (sshd) password authentication not working

Thanks. With UsePAM set to yes, password authentication is back. This
happened after the openssh upgrade to 5.4p1

However, public key authentication stopped working -- ssh does not ask
for the passphrase and falls back on the next available method.
Permissions on ~/.ssh and authorized_keys seem to be fine.
