Upgrading a MacPorts this old is probably pointless by now; you should uninstall the old MacPorts and all its ports, then install the current MacPorts and the ports you want.

Specifically, since the "sudo port -fp uninstall --follow-dependents installed" failed, skip it and go on to the next step.
