Is it generally okay to uninstall all inactive leaves?

That's more likely to be library deps though, and those will get quite
large for some ports. And removing one of those very probably *will* break
things (and MacPorts will warn you not to do it). There's just nothing to
be done about e.g. gtk-based ports having sometimes hundreds of
dependencies.
I will note that the build dependency issue is also present with Debian's
"apt-get autoremove"; there are no good solutions.
