If you are dedicated to actually using the 'patch' utility with a patchfile, there should be a way to get GitHub to provide that, but I don't see it. It is small enough (3 new lines and 2 changed across 2 header files and crypt.c) that you could make changes to the source by hand AFTER getting it downloaded, extracted, and patched by any pre-existing MacPorts patches, after which MacPorts won't notice your changes:
