Note that the documentation does say:

"You should create one patch file for each file to be patched, though  
it is permissible to use existing patches you find that patch  
multiple files."

I think we decided, though the documentation doesn't reflect that  
yet, that one patch per issue is also permissible. If you need to  
patch multiple files to fix one issue, then it's fine (perhaps even  
clearer) to put them all into a single patch file.
