Does qt4-mac implicitly request for +quartz variants of dependent ports?

The only reason they would ask to be done that way is if something else was already built that way. This doesn't necessarily mean it's from variants but because they're checking which files were actually installed.

Some ports build differently based on what is available on the system, which means something else built for quartz at one point in time and now the rest of these are noticing that and trying to as well.
