 What version of mysql is it talking to? Is mysql running on the same
 server or a different one?

 You could try rebuilding php5-mysql with a different variant. By default
 it uses the mysqlnd variant, but you could try the mysql5 variant instead.
 To do so:

 {{{
 sudo port install php5-mysql +mysql5
 }}}
