 Sorry, I was wrong.  startx checks the locks to determine what display to
 use.  The server itself creates (and removes) the lockfile.  Look in
 os/utils.c, specifically at LockServer() and UnlockServer().

 /opt/local/etc/X11/xinit/privileged_startx, which does not exist…

 Well that would be problematic, since privileged_startx is what sets up
 permissions in /tmp and your font cache.
